Sheltering Arms Adult Day Center (ADC) provides cost-effective community oriented center-based services to elderly participants and their families. The ADC serves individuals with third and fourth stage dementia that significantly impacts the activities of daily living. Our Senior Companion Program (SCP) provides in-home respite for caregivers. Aging in Place will enable the ADC to collaborate with SCP to increase the provision of in-home services, and to expand our existing telephone reassurance program to include calls on weekends to homebound isolated frail elderly in their homes. This project provides comprehensive services to seniors and their caregivers to prevent premature institutionalization.
