This project supports UWSEM’s objective of providing a rudimentary level of individual and family well-being and support efforts to move toward self-sufficiency. The target population is people with disabilities; this is a population which is inherently vulnerable to social and economic marginalization and that as a whole struggles to consistently meet their own needs. As such, this population is extremely susceptible to either falling into an acute crisis state or simply living in a chronic state of crisis.The agency will provide services and supports in three key areas of life: basic needs, community access and person centered planning.
