Vista Maria proposed to reduce systemic and individual barriers for youth aging out of foster care and residential programs as they transition into adultood by Improving Delivery of Services. The project was developed using nationally recognized assessment tools created by Casey Family Programs and Daniel Memorial Institute. Program activities are implemented using a partnership of agencies.
